Intolerant to Cow's milk (but not goat's), possible familial allergies to peanuts.

No.

Not really.
4.Is there anything we need to know about your child?s appetite and eating habits that might assist us?
I am aware that he eats more than the average adult. I do feed him breakfast (eg two weetabix) and he will need another bowl of cereal and toast with you. Second helpings at lunch is not uncommon. He will eat twice as many sandwiches at tea-time as any other child then ask for extra pudding. (BTW, nursery are fine with this and told me about how much he eats, it's not a rule from me!)
5.What types of foods do you want your child to eat often (several times daily)?
Healthy food that is requested. I am fine for him to have watered down apple juice with a meal, but not in between meals. He will request chocolate all day but I'd rather he not be given that all the time.
6.What types of foods do you avoid or not give your child to eat and why?
No nuts or cow's milk - allergies. Very little chocolate due to hypersensitivity to caffeine. I'd prefer him not to have sweets as he enjoys fruit and that's healthier.
7.How often do you give your child treat foods? What types of foods are these?
Never. Food is not a treat. He may have what he wants when he wants (exceptions being things mentioned in 6). We do encourage healthy snacks but we don't do treat foods.
8.Are you happy with your child?s diet? If not what concerns you most about it?
Fairly happy. I'd prefer it if he put on weight given how much he eats, but that's not something we can change.
9.Do you know what servings of each food type your child should have? Do you know what constitutes a ?serving??
Yes I do - however DS will eat twice as much as this easily. So I give him larger portions: should he start to have a weight problem then we'll look at cutting down portions to "normal".
10.What do you feel affects your child?s food choices? (E.g. family eating habits, food advertising, other family/friends, etc?)
Hunger and what he sees - he tends to see fruit more than anything else so eats more of that. Finger food is always preferred for a snack so he doesn't have to stop playing, but he refuses to finger feed for meals.
11. Does your child help choose foods for them to eat daily and/or do you cook with your child?
He chooses his fruit snacks, but eats what he is given. He does pick things in the supermarket when I take him there or request a sausage roll when we pass the butcher's, and we do sometimes cook together when we make cakes, but not the main evening meal.
