That's disgusting!
My DS2 is 18 months, firstly, he's not talking yet nor does he make a lot of babble or general sounds. He's a very quiet child. However his understanding is so good. Being verbal is not everything! (My DS1 has a severe speech and language delay/disorder)
Secondly, premmies are prone to anaemia (amongst other things) if you haven't already then add an iron liquid supplement (you can buy it over the counter at the chemist) My boys paeds recommend the full iron supplement over the multi-vitamins. It might also be worth requesting a full blood count to check on everything too, I had no idea either boys were anaemic until a blood test for something else (the Dr threw in the iron check just because we were already taking blood!) His stores were 3, Dr said they should be a minimum of 10.
Also my DS1 has chronic glue ear and re-occuring infections, the only way I know this is because we keep having hearing tests! He doesn't pull at his ears, doesn't appear unwell, no temp nothing he is completely ordinary! He once got a severe infection in both ears as well as a severe throat infection, was on antibiotics for a few months to clear it up. I only took him to the GP because his breath smelt! So don't feel bad, it just shows you have a tough cookie there
Other than that I have no further suggestion, you are doing a great job, don't let a crappy GP get you down

Massive  it's hard when someone undermines your parenting skills that much, you're doing a great job
Can you try explaining to the other dr's that it is a young child and you are really concerned? Some, if they are decent people will fit kids in because of the potential 'risk' (sorry can't find the right word to explain) to the child's health. Also do you have photos that would show the difference in colour? This might be helpful for the dr as well, and yourself so you can see the difference (ds was jaundice when born but I didn't see it until I was looking at photos itms)
Good luck and I hope you get some positive answers soon

Wow, I am shocked by how rude that GP is! Just wow! I hope that he's all ok now. In regards to speaking... ummm Elijah is 21 months and he doesn't speak... well he does in his own way but he pretty much only really says Ta and Pa but he communicates VERY well using his own forms of communication... which include grabbing my hand and dragging me to where he wants me to be to show me what he wants.
