I just want to throw in the towel..
Are you feeding on demand or at certain times? I found with DD2 if she was super hungry, so she'd slept longer than usual or I'd stalled a bit feeding her (hanging out washing, etc), then it was hell trying to feed her. I really had to feed her at the first signs of hunger, or just before!
So maybe try feeding him smaller amounts more frequently. I'm not sure on the recommended 'timing' for FF but maybe try 2-3 hourly feeds during the day and just offer less at each feed so he's still getting the same amount of milk just more frequently. Hunger is awfully painful for reflux babies.

I'm putting my bets on silent reflux too. And DD2 did the same thing even if I thought she'd fed soon enough. It wasn't until my LC told me it was the reflux and hunger making it worse that I could get a bit more of a handle on it. Remember that babies often drink 2 hourly so that could help :dunno:
When DD2 had those sort of 'attacks', I'd walk around with her to calm her first then try to feed her in an upright-ish position while still wandering around and it really seemed to help. I had to get away from too much stimulation such as the TV, bright lights/sunshine, visitors and even DD1 at times. I'd just take her into the bedroom until she settled. Once she'd gotten a bit of milk she settled quickly and fed well and I could sit back down with her. I was told by my LC and the GP it was typical reflux.
