Prama, I have used Motilium with great success. It works best though in conjunction with the good old supply = demand situation. Basically, feed baby as often as you can, somewhere around 8-12 feeds/24 hours. Obviously you're still pretty sore ATM, so feed as much as you can possibly manage, building it up as things improve. Don't stress if that's not alot at this stage. Certainly express where possible, but your baby will do a better job of draining the breast, so is better at building your supply up. And feeding instead of expressing saves you all that sterilising!
To start with, I really relied on the Motilium, without adding the extra feeds. It helped somewhat, but was not the magical answer I'd hoped for. It was only when I started feeding more frequently that my supply really increased. I think you can expect it to take up to two weeks to work. What dose are you taking?
Hopefully, once you are through this rough patch you can SLOWLY wean off the Motilium, and find that your supply is still great.
I would really recommend seeking the advice of a LC, if you haven't done so already. They will be able to help you better with all your questions.
Don't doubt that you are doing a fabulous job hun. We had months of problems with BF, but I can honestly say that it was well worth perservering. BFing my DS now is a wonderful experience, I am so glad I stuck it out :D Don't expect that Motilium will fix things overnight - it won't. But it might help, and with the hard work you are putting in I'm sure you will succeed and overcome these hurdles :)
Good luck, and don't forget that we're all here to help!
Kristy J, as far as I'm aware, Motilium has been used to increase prolactin levels for years in NZ, long before we were using it here. Just thought that might help you in your search.

Kristy J, you need a long talk with Rodney Whyte at the Monash Medical Centre drug information line (03)9594 2361 - He's the head pharmacist. also check Thomas Hale's drug info website.
But, more than that, you need to attend an australian Breastfeeding Breastfeeding Education class and see an LC to debrief about your last experience and get breastfeeding off to a good start. We don't NEED drugs to breastfeed, and nearly every woman can breastfeed, even twins. If you are the one of the rare ones who has a pre-existing condition which might make a full milk supply possible - best to know upfront (note - it's not always possible to know up front) If, like many of us, your poor breastfeeding started at birth, there are lots of simple things you can do to ensure breastfeeding twins goes well!
Prama, I know you have seen LCs before, but I think it would great to get a private LC would could look after "just you". It is a small investment in a happy breastfeeding future. I would be happy to suggest some-one in your area. Motilium acts by raising your prolactin levels. Prolactin is the hormone that makes milk. It will really only work if you also remove milk from the breast - otherwise, even though your prolactin levels are high - your body is saying "no more milk needed"
Ideally a newborn baby will feed 8-12 times in 24hours. but go gently on yourself! You may need to pump sometimes are give a bottle if your nipples a rest. But, seriously, see and LC

Thanks Barb, I'm going to see an LC at the Family Care in Parramatta tomorrow. The Early Childhood Centre put me in touch with them.
We're taking the nipple shield and formula etc. and we'll see how we go. I've bf in the early morning feed and also this morning with the nipple shield. Nipples are sore but so far ok. I'm trying to build up my confidence again by having more frequent bf feeds, but at the moment, some formula in between.
Thanks again

thanks barb....
I'm not sure what i will do yet.. i saw a LC while B/F my DS and she was a real B***h, she was just no help at all, one of those judgemental types. pushy and closed minded. in the end i did it my way. but quit a few weeks later my suply was so low i was dry in 48hours. anyway ill will talk to the association on the phone. but im reserved in seeing a LC.
i have done more research too on motulium turns out it is banned in a lot of countries most of their reasons being that some studdies show it may cause problems in babies as well as a number of problems for mum too. I guess its like presertivies, Australia is slack in banning the bad presertivies too. its all about money. I don't think i would take it.

Kristy, I'm sorry you had a bad experience with the LC when you were last breastfeeding. But most of them are not like that, and will offer you fantastic advice and support. I know the woman I saw was lovely, and really helped to turn things around for us. I think most people would say they had the same experience.
Don't try to do this alone - there is no need, there are people out there who can help you get your breastfeeding experience off to a wonderful start. Certainly contact the ABA, but really have a think about seeing a LC. With the right person, I think you'll be pleasantly surprised :)
