need advice multiple m/c, clomid, clexane and progesterone

hi everyone, i hope this is the right place to put this

sorry about the long title, a quick background, i have pcos and have two children who were conceived on clomid, in nov i m/c at 9 weeks from a natural pregnancy, since then i have had another 2 early m/c's (all natural pregnancies). so i have been to my ivf specialist and his plan is to start me on clexane and progesterone pessaries for my next pregnancy (and maybe clomid), im seeing him again at the end of the month (he wanted me to have 2 months off to settle my body down), so my questions really are when do you start on clexane and pessaries ? is it before ovulation, after or once a pregnancy is confirmed and what do they all do ?

Clexane works to reduce the ability of your blood to clot. One of the major causes of miscarriage is clotting problems, which is why it can help to use clexane for a pregnancy. Different protocols exist as to whether you start at confirmation of a pregnancy, ovulation, or the start of the cycle. You would need to ask your doctor for more information on that one.

Pessaries - they are simply used to help lift progesterone levels. Usual method would be to use them from ovulation and then continue once a pregnancy has been confirmed. Low progesterone levels are often found with pregnancies that miscarry, so sometimes increasing progesterone levels can help. Be aware that the discharge when using pessaries is fairly icky.

Have you been tested for autoimmune problems that may lead to miscarriage?

thanks BW, i have had the autoimmune tests done after my second m/c but nothing unusual showed up on them, i have changed to a new specialist now and im having all the tests run again next week, it just seems strange to me that i have my first 2 children with clomid and no problems but the "natural" ones have miscarried so i did wonder whether clomid "helped" my other hormones get stronger or that whether it was just coincidence

Clomid certainly can work that way, so it may not be a coincidence. Clomid can result in a better or stronger ovulation with higher levels of hormones which can make the difference for some women.

great thanks BW, the specialist basically said you don't want clomid you might have twins ! but i thought it was too much of a coincidence, i thought maybe with the pcos the clomid sorted out other hormones too, i'll definitely be asking for clomid again then !

Hope you don't mind me sneaking a peak at the answers you got minniemouse: i've just had my second m/c and my dr is also talking about putting me on clexane and progesterone next time (even if no explanation for my m/cs shows up on blood tests). Interesting to see that progesterone might be needed from ovulation. Thanks Butterfly Warrior.
