ending a lease- what to write in letter?

So we bought a house (yay!) so no more renting for us...How exactly do I word the letter that we send to the RE of our intentions to leave? We're on a month to month so we only have to give 28 days notice right?

I tried to google a template without much luck, can anyone help me out?

Please be advised that we will be vacating the property located at.....
We are hereby providing our 21 days notice. Our vacation date is.......

maybe check if your R/E has a "Notice to Vacate" form. if we wanted to leave the property we just had to fill out the form, sign it and give it to them, no letters required.
