fears about episiotomy 2nd time around, sorry it's long!

as birth is getting closer, i'm starting to worry more about the state of my perineum!
after a (relatively) easy and straight forward labour with my DS, i had an episiotomy in preparation for assisted delivery,tho i was able to deliver him myself.
my problems began when the EMS midwife was concerned that my stitches had opened at home and sent me back in to hospital to be checked. the dr, who was the one who performed the procedure, said everything was fine and sent me home. then my GP sent me back again at 6 weeks as it wasn't healing. again i was told it was healing internally and everything was ok. essentially it took 9 months to close completely on the outside, and healed with what i would describe as a tuck in it. its not painful and i can't feel it during sex.
what i am concerned about is what might happen this time. am i better to prepare for a another ep? do they usually do one if you had one first time? is it still ok to tear naturally if poss? i do have an appointment with midwife on fri so will ask her, but any advice would be appreciated. thanks
