3 yo - vomiting, no wee, wont take fluids, 39 temp - PLEASE HELP

Like the title says my DD is three and for the last 14 hours she has been vomiting, hasn't weed, I can't get her to eat anything (which is understandable considering how crappy she feels), she can't keep any fluid down or even panadol. Anytime anything touches her mouth the poor little thing vomits it back up within minutes. Her temp is now at 39 and climbing because I can't get panadol into her. She wont let me strip her off as she is too cold (fever shivers). She is pale and lethargic and hasn't moved off the couch all day except to vomit in the toilet. She's napped on and off all day - definitely not normal for her.
On the upside her mouth is still moist and her capillary return is still brisk so she shouldn't be dehydrated yet.
Should I get her checked out? I don't have access to a car for a few hours though.
What can I do to help her??

if she can't keep fluids down I'd be considering taking her to the hospital...maybe even give them a ring and see what they suggest? she might not seem dehydrated yet but in little ones it can happen quickly, you're better off being able to get fluids into her before she does get dehydrated. hope she feels better soon, must be awful for all of you! :hug:

I'd take her in & have a bag packed prepared. DD was the same last year & ended up in hospy for 5 days.
I couldn't get panadol or ab's into her. She was still pretty moist in the mouth, but was getting worse quickly.

Take her in, even though her mouth might still be moist, the fact she is not weeing means she is dehydrated to some degree. It will probably be much harder to get a doctors appointment at this stage and I really wouldn't leave it for tomorrow.

Our 12mth old had 39 temps 2 weeks ago - no vomitting, but nothing would bring them down. Doc said kids 3 and under cant regulate their body temps as well as adults/older kids, which is why its important to get them checked out when they do have high temps. So yeah....i would go as soon as you can get a car....

Definitely take her into the hospital...if you don't have a car then ring a friend or call a taxi. It's better for her to be seen in daylight hours rather than wait until later when the ER will be filling up with after-hours problems. :hug:

Do you have a GP who's happy for you to call? If you have a good relationship with them that might be a good idea. My GP always says they are ok as long as they have some 'spark' about them. If they are so lethargic that they don't respond to you then they are headed towards or are dehydrated. I have found this to be a good guide for me, but I imagine you are better qualified to judge it than me (your are a nurse, aren't you?).
Is there anyone who could drive you to the GP or to the hospital? Can your DH knock off work yet and take it as sick/compassionate leave?

BUT, for your DD...

Fever + rigours + nil wee + nil intake + vomiting + lethargy = hospital ED. Don't panic, you've got time since she still has moist membranes, but I'd be getting her checked out. But I'd call whomever you need to get that car back quicker.

I'd be disinclined to strip her off because of the rigours. If she is telling you she's cold, I'd not wanting to cool her down as the shivering will spike her temp further. Instead, try to wash her face, arm pits, back of neck with a cool flannel (not cold).

Keep pushing the fluids even if she brings them back up - some of it must be getting in ;)

Have you got panadol supps at home? They work better than oral AND don't rely on her being able to keep them down ;)

Do you know WHY she is feeling like this? Has she got an infection? Been exposed to gastro recently? If it's infection, they might need to consider IV AB's.

Wish I was closer to help you out - even to watch DS so you can take DD to get checked out.

If you can, call a cab and get your baby girl to the ER. As everyone has already said dehydration happens very very suddenly in small children. Hopefully by now you are on your way to hosp.

DD went three days with temps over 39, we couldn't get her to take any fluids, she wasn't vomiting, but she wasn't eating or drinking anything either, we had tried everything to get her to drink, we made the call to take her in because she would cry and there were not tears, so dehydrated and she ended up being admitted. We ended up having to resort to suppositories in hospital and when we came home as it was the only way to get her temp down, is there a way you can get hold of some first, it may help get her temp down and make her feel a bit better.

Even with the fluids just syringe a few ml in at a time, keep away from anything dairy as well.

Thank you all so much help/suggestions. I took her down to our ED department. I went over everything I posted and added the diarrhoea which started in triage room. The triage nurse was lovely but the dr was an protanising a$$. Apparantely I'm just a mother who is over reacting!!!!! There is nothing to worry about. We were told that we just need to get fluids into her and her body will sort itself out when it's ready. I knew I hated going to ED for a reason!!! They wouldn't even give her PR panadol or an antiemetic because her body is doing this for a reason. I KNOW THAT BUT CANT YOU MAKE HER COMFORTABLE!!! Anyway my gorgeous little princess is curled up on the couch, grizzling "oh mummy me sick so yucky" it's breaking my heart. Her temp is still over 39 but I have at least have managed to get four teaspoons of diluted juice in her and she has kept it down. I'm watching her like a hawk for a wee.

Oh Dan that sucks! That would have made me seen red and I'd have demanded he do something. There might be a reason for it but there are also risks (as you know) for those combined symptoms.

Keep watching her and if you're not happy, go back or ring your GP.

I do know they dislike giving maxalon in vomiting children as it isn't always effective.

Have you got some panadol supps? May be a bit tricky if she's got the runs but give it a go. Poor poppet. I totally sympathise. DD was whimpering and call for me in her sleep over the weekend and it broke my heart.

Liviam, I don't want to alarm you, but if I were you, I would take her back.

She sounds to me like he is getting very, very dehydrated.

If she is no better by tomorrow morning, definitely take her back and agitate until they admit you and rehydrate her. Does our nearest hospital have a proper paediatric ward? If not, how far away is the nearest hospital that does? Because I have to be honest, if you presented to me with your history, I would have either put in an IV or put down an NG tube and started rehydrating her by now.

Don't be afraid to take her in overnight if you need to, either.
