I reckon you could also email back - and educate her as to the real cost of those other great 'choices' in hospitals...and how much THEY cost the taxpayer! Quite aside from the insurance scenario (or lack there-of, as mentioned in the original email...a very observant psych, indeed...).
ETA:
She might be the organisational psych doing some overtime?? Maybe the receptionist passed the email onto the org psych? Maybe she practices part time? Maybe she's still under supervision because she can't read a brief...and respond appropriately...
