ultrasound results need help?

hey

i just got back from having a ultrasound done as i have been having alot of discomfort and i am having a hard time understanding what the results mean, i had a very unhelpful doctor today

ok this is what the results say, word for word

the uterus appears unremarkable. At this stage the endometrium measures 11mm maximally and there is a little fluid in the endometrial canal.
The ovaries were identified and there is a dominant follicle on the right side measuring 26mm maximally. This follicle contains some internal echoes which may be due to haemorrhage or infection. There are no other specific features although on the left side the ovarian follicles are somewhat peripherally positioned. There is no evidence of free fluid.

i ovulated about a week ago and have had discomfort since then.
so my questions are
* should there be fluid in the endometrial canal? if not what does this mean now that there is fluid there?? does this exlpain why sex is so god damn painful?
* so did i ovulate properly? if so then why would there still be a dominant follicle on the right side?
* what does internal echos mean? and the haemorrhage or infection is this bad?
ok last question what does it mean about peripherally positioning?

all my doctor told me today was that yes i have a cyst and if in a couple of weeks i dont feel better maybe i should think about seeing a gyno?
does any of the above things mean bad things for my future in ttc, i am going to be starting ivf soon will this effect anything with that??

very confused and just looking for someone to put it in simple words

Wow, what a report, sorry i don't have any answers for you. But I do think you need to speak with a qualified person who can answer your questions if your Dr can't. When you say Dr I'm not sure who you mean. If you were seeing a Fertility Specialist they could answer your queries and give you further information or your IVF clinic if you move in that direction. My FS is also a gyno and an obstetrician which helps.

In between time you may like to move to the LTTC Assisted Conception. There are some ladies in that thread who have a lot of experience and may be able to provide you with more information.

the doctor that i seen was just a normal gp, i have my first appointment with my ivf doctor in 4 weeks and was going to ask him if he could tell me what it all means, but i am still very uncomfortable down there, i feel sore in the right ovary when i am walking and laying down, i cant put my legs down straight i have to have my knees up while i am sleeping, which is a pain in the bum cos i love sleeping on my side lol
i was hoping that someone here might of be able to help me with the results as i really dont want to have to wait 4 wks before finding out what it all means and i really dont want to go back to gp as my normal one is away and he has a fill in who is very rude and seems like he just doesnt want to be there iukwim?

I'm sorry you had such a twat of a dr to give you these results!!

I can only give you uneducated guesses as answers to your questions...
According to my book conception, pregnancy and birth by Dr Miriam Stoppard the endometrium (your uteris lining) thickens as your cycle progresses. The endometrium becomes spongy and thicker when it prepares to recieve a fertilized egg. So maybe a little bit of fluid is ok?? To help make it spongy?

Dont know about the painful sex, sorry.....

as for the follicles, the follicle releases the egg, but remains in the ovary and if the egg is successfully fertilized it turns into a corpus luteum and sustains the egg while it implants into the endometrium (uteris lining) which is nice and thick and ready and waiting.... so i would say this is the follicle that released an egg when you ovulated a week ago. The follicle goes through changes and flows with your cycle, but i am unsure if the "internal echoes" should be there??? Maybe thats why the dr said if it doesnt feel better in a few weeks see a gyno, becuase it could all sort itself out easily, and may actually be a good thing??? Hmmmm...

It would be too hard for them to pick up the egg at the moment, it could be still waiting for some spermies or it could be travelling to where it needs to be, and in a few weeks, you will either find out you are pregnant or you will get a visit from AF. But like the dr said, if you are still in pain then see a gyno.

I have NO idea what the peripherally positioned means.. sorry to not be much help, i hope you get some answers from your FS, he or she would be the best person to answer your q's.

The follicles in the left ovary being peripherally positioned just means that they tend to be found around the edges of the ovary. I believe this is saying that it appears to be somewhat polycystic in appearance. This in itself does not indicate that you have PCOS, though.

I think the internal echoes just mean that the follicle is filled with fluid - it's just describing the way the ultrasound waves respond inside the follicle.

As for what it all means in relation to painful sex, you'd need to see a doctor for that. I would like to think that if the GP wasn't too concerned about what the report says, then it's likely to be nothing too troublesome. Make sure you take the report with you when you see the IVF doctor.

There's always the problem of reading ultrasound reports - they use scary-looking med-speak which often doesn't actually mean anything significant, it's just a fancy way of describing what's there.

you have eased my mind alot

just dont understand why its so painful though???? i have had plenty of cysts in my time and not one of them them have dropped me to my knees like this one. doc has me on penedine forte for the pain. everytime i stand up straight it feels like gravity is pulling my ovaries out, its sort of like a ripping feeling if that makes sense lol.

bw i have PCOS, found out when i was 17 that i had it. its been a pain in the bum for my fs lol i ohss on 50mg of clomid its going to be interesting to see how i go with the ivf drugs?? i know that you have had such a rough time with ohss you poor thing, i really hope things go smoothly for you. i am always reading up on you and seeing how your going, i truely think your a strong women.

well hopefully i will be able to sleep better tonight, the old brain just wouldnt stop ticking over worrying about what was going on lol

Mummy2chloe, it finally looks like my FS has got things right for me in regards to the OHSS thing. Part of it is me being incredibly strict with diet - low GI, low carb, no sugar, high protein. It's really obvious that I get much better eggs when following it, and it now looks like being super-strict with it is helping head off the OHSS. But again, PCOS means your response to puregon (and all forms of ovarian stimulation) can be really unpredictable and variable. I've heard of women having a poor response to one particular dosage, then later getting OHSS with a lower dosage, all because of the damned PCOS!

Afraid I really can't help with the cyst - I've never really had one at all.

Just read your post been away but a dominant follicle is one that hasnt ruptured a dominant follicle is the one that is ready to realse the egg so maybe you ovulate later in your cycle than you realise, also DTD would be painful due to the size of the follicle usually they rupture at 18 to 22mm if yours is 26mm i would say it would be cuasing some (if not alot ) of discomfort. Dont really know much about the fluid side of things, but i hope ive helped a little. If it keeps up i would maybe see someone about a possible pregnyl injection to make you ovulate. Can i ask are you temping? as that how you know you 'O'd last week?

thanks for your help, i dont temp but i do opk and it was the strongest positive i have ever had, in the last 5yrs i havent been able to get a positive, only faint lines but this month when i done the ovulation tests the positive line was way darker then the test complete line plus i had heaps and heaps of ewcm.
