Submucous Fibriod by Hysteroscopy

Has anyone had experience in this area? After being told again and again that this potentially affect my chances, I thought its time.

The fibriod is not big, its 5mmx5mmx7mm, with less than 25% protruding into the uterus cavity, which means majority of it is in the uterus wall. I don't think the uterus is distorted because of the fibriod szie. Hence, my reluctance to go through a surgery in the first place.

FS suggested Hysteroscopy which I am happy with as there won't be incisions. But I also worried how effective that is going to be due to the type of this fibriod, there is only 25% inside of uterus cavity, so that's all he can remove.

Will the removal scar the uterus? How would cutting off the 25% of the fibriod improve the blood flow around uterus etc?

Has anyone done this before or in the similar situation and had a pleasant result?

I'd love to hear some stories to push me through the line..:p

Sorry to hear you've gone one of those lumps :( Unfortunately, these are sooooo common, and half the women don't even know they have them until they are pg and and they pop up on the u/s. I had a 7x7x8 cm fibroid that grew to 13x14cm during my pg last year. It grew so large due to the pg hormones and the IVF drugs.
My FS said that although the fibroid was on the large side, it didn't protrude into the uterine cavity so implantation would not be a problem. He was right, I got pg on my first cycle, BUT, 4 1/2 months later, the fibroid put way too much pressure on my uterus and I went into labour ar 20w &6d.
I have since had an open myomectomy op in Jan this year, and the 'dead' fibroid came out without a hitch. The fibroid 'died' a week after I delivered my baby, so removing it was really straight forward as there was no blood supply anymore.
If I was in your shoes, I would get some more opinions on what the best possible path would be for you, as doctors tend to have very different views on fibroids. It is such a grey area. In most cases apparently though, a woman can have fibroid/s and still have a successful pg. It's when the fibroid/s begin to protrude into the inside cavity, that implantation can be a problem or if it is so big (like mine was) that it can sometimes put too much pressure on the uterus. Oh Beata, so sorry to hear your loss...I hope this upcoming FET will fulfil your dream especially after the fibriod gone.

Mine was the submucons one, very small but 25% into the cavity, that's why its tricky. Because its so small it didn't alter the shape of uterus or anything, but reading your story, I just don't want to take any chance down the track. I have very much decided to go with the surgery especially my FS has done a lot in this area. But just thought to hear other people's story, thanks hun for your support.

I just wanted to tell you that my FS believed that removing the already 'big' fibroid prior to the IVF treatment carried more risk than leaving it alone. He believed that the uterus wall might sustain too much damage after the op, to carry a pg to term or even get pg. I really don't think he thought I would lose my baby, and after my loss he said he wanted to remove the fibroid asap as now that risk of it happening again was very high. So out came the fibroid, and he was really happy with the op, and I've had a u/s earlier this month and the uteus wall is healing up nicely. But I now have the fear of the wall bursting or something going wrong when I get pg, thinking back to what he said in the first place about the wall sustaining too much damage after removing the fibroid. I sometimes think I just can't win!!!(sorry, having a whiney day :rolleyes:).
I hope everything goes well for you hun, like I said before if your fibroid is poking into the cavity, the little embryo might not want to attach itself. Just please remember that my fibroid was probably 10 times bigger than yours, and although it didn't poke inside the cavity at all, it was growing mostly on the outside and put a lot of stress and pressure on the poor little uterus.

Oh beata, I know the 'you can't win' part, no matter what we do, we will worry....but you've made the right decision of clearing an obstacles, as far as you give enough time for uterus to heal, there should be any problem for future pregnancies.

I've already decided to go with the op and get it done and over it, even it didn't pose any threat toward implantation, I'd hate to think that it will potentially risk a pregnancy. I so look forward leaving all these behind. All the best hun...

Sorry I didn't see this earlier. Yes, I had 2 submucosal fibroids and I had them removed in December. It didn't hurt but I did bleed (very slightly) but for about a month afterwards. I would still have it done if I were you. I was very hopeful this was the problem and we would be preggas soon after but not so lucky yet. I have done a lot of research on it though and there is strong evidence that submucosal, even without distorting the cavity, significantly reduce your chances.

Be a bit careful when you read stuff on this. General fibroids are not thought to be an issue but submucosal are different (only 5 % of people have this type of fibroid). I would absolutely have it out.

Just out of interest, were yours big and were they distort uterus cavity? Mine was only 25% into the cavity which means 75% is still intramural, so I am just wondering even Hysterocopy will still only get that 25% inside the uterus, will the other 75% stay in or will grow inside again down the track? Any insight on this? I guess that's tricky part of having this kind of fibroid, 5%, how special are we? ;)

A month bleeding sounds a bit scary, did they say the reason why or it is just normal post op?

Mine were not big. In fact, on ultrasound they had difficulty even seeing one of them and asked me why I thought there were two (my FS saw then during hysteroscopy). The other one had been seen on ultrasound about a year before but they had previously told me that because it wasn't distorting the cavity they didn't think it would cause a problem. It wasn't until 4 failed IVF cycles with good embryos that I had a laproscopy and a hysteroscopy that was when my FS saw them. FS told me they reduce your chances of getting pregnant by about half even with no cavity distortion.

The bleeding was really just annoying. It wasn't proper bleeding, very light and, TMI - almost watery. I think it was just healing. We weren't allowed to try for 2 months afterwards which I was initially frustrated with but it was actually sort of nice to have a forced break. It was a relief not to have the pressure of "trying".

I have another fibroid which is quite big but on the outside of my uterus and they haven't removed that because the general consensus is that the ones on the outside don't cause issues.

Oh - I forgot to say that mine were similar (only part sticking out). FS said that it didn't matter if the bit in the wall stayed. I think eventually fibroids do grow back and new ones also grow but takes some time.

seems we are so similar Bridget, I've had 5 stim cycles so far as well. I also felt that I've wasted totally stunning 5-day-blasties. Its great to know that you've experienced on fixing those damn fibriods and show me what to expect.

Did you have hysteroscopy and Lap at the same time, I am just wondering so far my FS is just talking about hysteroscopy and if that alone will be able to deal with the problem. I guess the reason you did both in the aim to check everything in order? How long the recovery time needed after op in order to get back to work?

I guess the two months off not TTC will be something to look forward to....I am not a 'spring chicken' but I guess the mindset 'clock is ticking' is just adding extra stress to the situation and I already plan to truly enjoy this 'off time' to nurture the soul, much needed.

I am so pleased to find this thread - and hope you don't mind me jumping in. I have a Intramural fibroid. My FS has known about it, but a laparoscopy in Jan this year revealed it's quite a bit bigger than she thought; grapefruit variety ? she showed me the photos.. It's not impacting on the shape of the cavity wall, but with 6 failed emby transfers, she thinks it's time to take it out. She says this is a pretty big operation.

Bridget: how was your recovery. My FS is saying 2 weeks off work + six weeks no lifting etc. This op has me a little worried.

Jaggles, welcome to the thread...although mine was a different type from yours. From what I read, intrumural normally doesn't affect fertility and most people don't even know its existence. But given the size of yours, its probably out than in I guess.

Have you consulted with your FS if you can get it out by laposcopy or have to cut you open? As the recovery time will be significantly different. Good luck on finding the answers, I know how scary it can be....

My FS has done every test under the sun to try to work out why my embryo transfers aren't working - so the last thing she can see that is interfereing is the fibroid. it's all a bit of a nuisance really - and as my DP says, the old fashion method is much more fun.

Girls, just to put my two cents worth, my fibroid was intramural which is the one inside the wall of the uterus, but because of the huge size, it could only be removed via an open myomectomy (5 days in hospital and 6 weeks recovery at home). Bridget, I know you said you have a big one growing on the outside which I'm assuming it's a subserous firbroid? Mine caused the late miscarriage because as the uterus was expanding, the blood supply to the fibroid was cut off and it started to go crazy which in turn caused the urerus to contract involuntarily. Yours could be OK on the outside as long as it doesn't put too much pressure on the uterus later on.
Jaggles, I would definitely advise you to get your fibroid out as after I had my miscarriage, I read somewhere on the internet that any fibroids on the bigger side should be taken out as they can definitely cause a m/c in the 2nd trimester. My OB said that the fibroid that I had was generally OK when pg, but later told me that in 1 to 2% of cases it can cause a late term miscarriage. Lucky me, I was the 1 to 2% of the cases of something going wrong.

Cuddlepie - I had a lap and a hysteroscopy initially (they discovered the endo and the fiborids and actually the ectopic pregancy from 9 months earlier still attached to the outside of my uterus) but they didn't operate on the fibroids because they had found too many things :rolleyes:

So, the next month, I had they fibroids out via hysteroscopy. It was fine. It wasn't anywhere near as bad as the lap. I only had 3 days off work. My operation was conveniently just before Xmas. Actually I had to beg to have it then because they had stopped doing most operations due to xmas (they like to be around for a week or so afterwards in case anything goes wrong) but if I didn't do it before xmas it was going to be feb. Anyway, it meant I really got to party over xmas and new year.

It's nice to find other people with similar issues. I didn't have any luck this month but I will keep you posted.

I'd also love to hear how you go with the op and I wish you lots of luck.

Bridget, thanks for coming back to let me know. I heard your news and feel so gutted for you. But again, its only the first (or second?) time after the op, so don't feel defeated hun, I know its hard, I hope the odds will be in your favour soon.

I will keep you posted about my op. Your post gave me so much relief, knowing its not that dreadful as it sounds. I am looking forward to leave all these behind. I haven't got the op date yet but expecting to be sometime later this month.

Just got the op date, which is 18th June next Thursday for a hysteroscopy to get the submcousal fibriod out. FS said 70% chance to get it all out but for some people it might need the second op until the rest protruding into the uterus to get it out.

I hope you only have to have it once. They told me the same thing when I had it done and I was lucky and they got it all the first time.

Just remember - the actual op wasn't much worse than an EPU for me and hopefully your experience will be similar

Had the procedure yesterday, all went well. Afterwards, my FS said he got most of it and just need to have an u/s next month to see how it looks. I hope I don't have to go in another time but seems like its a minor procedure, I probably was in there about 30-40 minutes max.

I didn't feel too sore, of course yesterday I was dosed up with Penedines and some effect of general and slept most of the day. Trying to watch some DVD but just got too tired. I didn't know I can sleep that much. There was a bit bleeding afterwards but I don't feel any cramps or pain which was good, later of yesterday those bleeding are very watery, so not as scary as I thought.

Today, i just so eager to get out of the bed but still found tired easily. I think the general is have more effect on me than the procedure itself. I am off the painkiller and feel fine, but just feel really light headed and nauseous sometimes. I hope it will be gone soon. Just taking it easy at home, lucky the weekend is on horizon.

Bridget, hope you are in the middle of your planning for the next step and would love to be there for your journey!

That's great news Cuddlepie. Just take it easy for a while. I was really tired after mine too. I guess healing takes a lot of energy :-). And thanks for asking about me. I'm on attempt no 8 and not feeling very confident. 3 out of 4 snowbubs didn't make it. so just one on board. I guess we've just got to keep running the marathon for now, I just wish they'd stop moving the finishing line.

Bridget, I have everything crossed for you for a sticky embie on board, you know it only takes one! I hope the marathon is going to hit the finishing line soon...;)

AFM, still have a bit light pinkish discharge going on (sorry TMI I know), I hope this is normal. Everything else is feeling pretty much back to normal. Do you think the op will affect you normal menstrual cycle at all?

CP - am so glad it's all over. I know how nervous you were. Am very glad you didn't have much discomfort...told you!!

I had a really short cycle (for me anyway- 26 days) in the month I had my op. And I normally have VERY regular cycles - 28 days on the dot almost to the minute. sometimes I think your body is really smart.

Hello ladies, a little update from me post op ...

My cycle returned very much on time (31 day cycle) which is a good news, although mentally I was prepared that it could be delayed. I was so glad its not painful at all and very much the normal length flow etc. So my AP said seems my hormones are behaving itself...

Last Friday I went in for a Sonohystergram to check if there is any residual or scaring post op, and it took about 40 minutes, its awkard but I am glad to say that they are having trouble of finding anything on the u/s and the gyn said without knowing my history, she wouldn't know there was a fibriod there. I was so reliefed that I don't have to go in again, YAY! :dance:

So now just waiting some formal report to be sent to my FS and he will give me a formal green light that all is clear. My next step is to embark on the next big thing of my journey trying 'immue suppressive therapy' adding to my IVF cycle, but I choose to wait for another month so we won't start until late Aug/early Sep. Just trying to enjoy a bit more of other things in life...
