thread: who decides?? 2day/3day/blast

    who decides?? 2day/3day/blast

    is it the Dr's decision or can i say i want my embies grown to blast/3 day/2 day??

    does the patient get a say at all??

    My clinic does two day transfers. I am not sure that is up for negotiation.
    But I think if you want to change the protocol you are on you should ask. It won't hurt and you may just get what you want! Is there any reason you want to change?

    I was told that it's all down to the embies themselves. My clinic prefers to transfer day 5's, but if there is one that needs the help of it's mum at day 3 they will put it in early..... I haven't actually been through it yet though.

    I think it depends on each individual clinic, mine does day 5 blastocysts. That said, I dont see why you couldnt ask for your preference, they will have reasoning behind why they prefer a particular day and can explain this.

    Diff clinics will prefer diff protocols as their scientists and techiniques will be more specialised in one area...each type of growth scenario requires diffeent techniques and cultures etc etc.

    For example here in melbourne MOnash IVF specialise in blast transfers whereas MIVF specialise and get similar results with three day tansfers.....all clinics should be able to grow to any stage however and you need to consult with your doctor if you want to change your protocol. At the end of the day it is really the scientists who make the final decisions on which stage to take the embryos to....if you have embies they are trying to culture to blasties and they are not looking so hot they may make a recommendation to the doctors to put them back at day three or somethin like that......so really it should be a collaborative decision based on all the data they have available including the succes or otherwise of your past cycles....

    In short, talk to your FS and see what they say....

    My clinic try to let them go to 5 days. My embies were pretty bad quality so mine went in at 3 days.

    Our FS told us what he prefered to do and why. We could of, if we wanted, objected but he is the specialist and so went with his advice (which has resulted in 1.5 kids from blastocysts so we're very very happy). So it was set as the protocol but I also understood the embryologist also had input once the eggs were in the lab.

    it's your eggs, your body so you definitely get a say... you can definitely question them as the decisions, at the very least you should be informed about why they think it should be day 2/3/5.

    My clinic's policy was to grow to blast as long as there were more than three embryos on day 2. It was always discussed vaguely with the FS before the cycle, then afterwards discussions took place directly with the scientists.

    In the incredibly sucky situation where I only had three embryos fertilise, the scientists rang me to ask if I wanted to attempt to grow to day 5 or did I want to transfer at day 3. We chose the day 3 transfer and got our boy from the cycle I completely wrote off in my head.

    I am hearing these days that some clinics will ONLY grow to blast. There's no option to transfer early, regardless of how many embryos there are. Personally, I'd prefer a clinic that's a bit more flexible. If you only have a few embryos, I'd be wanting to get them transferred ASAP, but that's going to depend a lot on how you do with growing to blastocyst - I had a LOT drop off between day 3 and day 5, but others do better...

    Besides... if you've always done it one way and aren't getting success, I think it's worth changing what you are doing just to see if it helps. There's no rule that says one way works better for everyone. This is more an art than a science. I know of people like myself who always did blasts, got nothing, changed to an earlier transfer and finally had success. I also know of the opposite where people have always done day 2 or 3 transfers and got nothing and who then changed to a blasts and finally got their baby... Part of the reason why flexibility in your clinic is important, I feel.

    Scary concept - the clinic my FS is at ONLY does blasts and DOES NOT update on the progress of the embryos between EPU and transfer. They don't even tell you how many fertilised... You can bet that if we ever go back for another child that I'm sure as hell not going to that clinic!

    Our clinic will do either. Basically you are paying for the treatment so what you say goes. Obviously the FS and embryologists will give you their recommendations. Most of the time is there are three or less embryos our clinic will do a D2 or D3 ET. Any more than that is usually a D5/blast ET. BW's advice is excellent. i cant really add more to that except that there is a lot of science and a lot less art that goes into growing a baby from IVF Good luck with whatever you decide x

    our fs usually does 2 or 3 day transfers we don't get blast options. but it is o.k
