Does the Dr keep a copy of CO form?

Need to get a copy of the Conscientious Objection form for DD1's kindy enrollment. I've already got it filled out and sent it off ages ago. Anyone know if my Dr will have a copy of it that I can just get off them? Or do I have to get it from Medicare? Or do I have to fill out another one??

I just gave family day care the medicare immunisation record print out that says "up to date" on it and the list of his vacs as he was partially vaccinated. Would they be happy with that?

My doc has always signed it then I posted it, so no they didn't keep a copy
