    Q's for non vaxers re the CO form

    Why do i need a CO form signed and where do i get one?

    The GP will have them, you used to be able to get them from a CL office too.
    You will need to show it in lieu of vaccination records for school/kinder/preschool enrollments and for CC/creche, and also, as per new legislation now, to Ikea Smalland!
    You make an appt with GP, they will discuss how foolish you are, how you are single handedly going to destroy herd immunity and will sign the form with pursed lips and much huffing, and you go.

    i printed mine off from on-line (maybe medicare office website?) and got mchn from local council immunisation session to sign it.

    So since DS isn't going to any of the places mentioned (kinder creche etc) I don't need one yet?

    You also may need it for CL records and the Immunisation Register.

    Also if you don't get one (I didn't get one for ages with DD) Medicare send you out letters stating your records aren't up to date as they have no record of vaccinations etc. and that you need to send proof if they have been or have CO form. Was something like that from memory.

    didn't they recently change the FTA/B supplement so you only get it if you are up to date/CO form signed for vax s?

    yeah, one of the supplements I think you need to vax or get the CO done.
    the reminder letters are annoying. I must remember to do mine.

    Yes. Ftba supplement is tied to being up to date with vax or having co lodged. There are three years in which up to 726 per year won't be paid at reco if not up to date. So even if you are not going to use child are its still worth lodging the form

    ok thank you.. I just got a letter reminding me DS's vax aren't up to date..

    BG - Is there a $726 that i should have received?

    ETA: i just looked through my records and it shows:

    FTB part a arrears payment, then FTB part B arrears payment supplement.. Is that what you're talking about?

    Only your gmfyb supplements if all your tax details are finalized. Lots of threads about it. Can't link from my phone though

    Yeah if you don't fill one out, you'll keep getting mail from centrelink saying you haven't had your immunisations up to date therefore payments etc can be affected or something like that..... not sure exactly but yeah we keep getting mail unless we do ours!
    Only just did DD2's 3 weeks ago!

    And at least by submitting a form then medicare knows who is making a decision not to vax and who is just not bothering
