    Question What information do I need...

    A gf told me that when you are at the hospital, they give you all the forms to fill out for medicare, family tax benefit and the baby bonus.

    My question to all of you is what information do I need to take to the hospital so that I can fill these forms in without having to hunt around for different paperwork. I would rather have it organised in advance to so that it is one less thing to worry about when I ahve this baby.

    She said something about an estimate of what DP and I earn... do I need group certs? Is it an estimate of what we will earn or what we earnt the previous year (as obviously these are going to be different).

    If someone could give me a list of info I need that would be fabulous
    or
    even direct me to a relevant link with these 'forms' I am supposed to fill in. Maybe I can fill in some dummy copies before hand and then just transfer the info onto the documents they give me at the hospital.

    I am just hopeless at this sort of stuff and will feel much better if I am prepared.

    the forms aren't available online sorry hun, but i can give you a brief rundown

    you'll need:
    existing c'link reference number if you've ever had payment (including youth allowance) - if you don't know it, don't stress - you can be searched for under your name
    Tax file number for yourself and partner
    if you've every lived overseas permanently, you'll need the date you left and date you came back
    if you've been o/s at all, i would take dates you left and when you came back
    if either of you were born overseas, the date you started living in Au, the visa subclass number you arrived on, the date you became an Au citizen (if you have) or your current visa subclass number
    an estimate of your 08/09 income and possibly 09/10 for both yourself and your partner - this will be broken down into taxable income for each of you, fringe benefits, any non-taxable income, foreign income etc
    medicare number (this may or may not be needed - make sure if you have two diff medicare numbers you decide which one your DD is to be added to)
    if you pay rent, how much you pay each week/month, the details of your landlord (estate agent/private landlord) including address for them. whether you have a current lease or tenancy agreement.

    that's the basic info you need. the only way you'd need more is if you have a more complex living situation...

    Just some advice! They will give you an orange 'Parent Pack' containg the forms. Fill it in ASAP and send it home with someone. Apparantley people have been stealing these packs and you only get one copy. If someone gets there hands on your pack they can claim the $5000 as nothing is filled in yet to say it belongs to you.

    the pack should include the form that will be signed to say your baby has been born that you use to register it's birth, one will be signed for c'link purposes.

    it concerns me that people would steal the packs! that's just sad...

    i would avoid having your actual documents on hand for anything that i suggested (best to take minimal stuff to any hospital visit) - just maybe have a notebook with it all in there - and when you go through the details over the phone with c'link, they'll let you know if anything has to be provided to the office - make note and supply it so that you don't delay your payments at all...
