For adults it's combined with tetanus. Not sure about the 4 year one, but there is a booster at 4, so it could be on its own.

No, it's combined with diptheria and tetanus, you can't get it separated.
I suppose you have to think again about the reasons you don't vacc. Why just get one vacc?
