    I think before you get yourself any more worried make an appointment with your DR, OB or midwife and talk this through.
    This will do 2 things. It will get you some answers and will make sure there is a record on your file so they can be aware for when you have your next child.
    Never be afraid to ask questions when it comes to your health and especially when it is dealing with child birth. The questions you are asking are normal after your last experience.

    yes i agree here as well, find out why this happened it may make u feel better about it , and they can talk u through it and if needed help you to kake sure it doesnt happen again or is managed better
