    Affidavit and family court question

Hey there, I know people in the legal industry can't give out advice, but I need to find out how to write an affidavit for a friend of mine who is going through the family court with custody issues. I have to be a witness for some things that's has happened while I have been there. So far I've written it out in numbered paragraphs, times new roman font, size 12 and double spaced. Have to go to get it sworn tomorrow, do I have to have it printed on an affidavit form if it's beig attached to my friends papers she is filing or what do I need to put on it in terms of personal information, headings, forms etc.
    Thanks

You need to Swear it yourself. It has limited weight being attached to your friends affidavit. The forms are on the Family Law Courts website.
