Something I have found to be super useful is slathering the area with cream - Sudocreme, bepanthan or zinc - and then covering the entire area with powder. The powder stops the cream going all over the nappy and getting lost.
My baby girl was teething and had gastro and her nappy rash was terrible. We cleared it up by they way I described. Good luck :-)

The most helpful thing for rash is air.

If there is ever blisters it should always be seen by a dr just in case it is nothing more serious than a rash.
