I am. For the first few weeks, I was staying home. I've just given her keys so I'll probably start going out when she's here. They just get on with the job. I'm usually on the 'puter "working".
I don't have a cleaner (ahh to dream) but a friend was a full time SAHM and she did, she just went out for the time the cleaner was there, i think she's even worked it around HER schedule like when her DD had swimming etc.
DP's aunt is a cleaner, she says most of her clients are at work or go out when she's coming, it's very rare she has someone actually around but she has a few older clients who just potter around the garden while she's inside.
I have just recently given her a key so that I don't have to rush home or wait for her to arrive, I was scared to give a stranger a key but figure after 3 years she is ok!!!
Occasionally we have had to change day due to kids being sick etc and it has worked much better on a day that we are all out all day at child care work or uni placemenets!
