You don't have to tell the ATO what you have or haven't claimed.
They are only interested in the number of days in the year that you were covered and the provider code(s) - if you'd changed providers you would have a code for each of them.
It's sounds like you just need the code for NiB and you list how many days you were covered.
You should have gotten an email from NIB with a link to your statement online, you should be able to log onto the NIB site to check statements anyway. My tax agent only wanted my membership number for my PHI to do my tax.
