    Default New to the site

    Hi my name is Cass and I am new to this site, I have being going through IVF treatment for over a year now due to having my fallopian tubes removed because of three ectopic pregnancies and also MF, I have a specialised appointment tomorrow to discuss the next course of treatment. I personally dont think I was stimulated enough last time and I wasnt happy with my progesterone levels even though I used pessaries and I also tried the crionone another time. Can anyone tell me if they have used the series of 3 injections instead of the pessaries and what were your progesterone levels like. We had ICSI and a day 3 transfer using crinone and our first FET was natural cycle and our second FET was with progynova and pessaries 3 a day. Also I am hoping you ladies can tell me whether it is better to do a day 2 or 3 transfer or a blastocyst transfer.

    welcome Cass -sorry you had to be here at all but I hope you find support like I have.

    I am sorry for your EP losses and loss of tubes.

    I used Pregnyl (3 injections)on my natural FET but they didn't check my progesterone -
    but it did delay AF by nearly a week - just enough to get my hopes up -on crinone AF arrived 12DPO.

    I had day 2 transfer on ET and FET - I want to ask about blast this time round.

    Cass - my clinic will only do blasts and claim this is why they have one of the best sucess rates in the country (SIVF). You will always end up with less embryos however. But I agree that if they don't survive in a controlled situation they probably wouldn't have survived inutero.

    I don't have much experience with progesterone levels, but I'm sure some other girls can help you more.

    Cass,

    I did 8 cycles altogether, beginning July 2004. Of these, 5 were stim cycles and 3 were natural FETs. I have no problems with my P4 levels, so my doctor never believed that I needed a medicated FET.

    Of my 5 stim cycles, one was cancelled due to poor response, but in all other cases, I averaged between 7 - 10 eggs. Our fertilisation rate was good, between 5-7 fertilising each time. Except for the 1st cycle where we only transferred 1 day 2 embryo, we always transfer 2 embryos at day two, bar the 7th cycle where we transferred 2 day three embies that had had assisted hatching done. None of these worked.

    For the last (8th) cycle, we decided on blast transfer. We collected 10 eggs, of which 6 fertilised, and we ended up with 3 blasts. Of these three blasts, 1 was a Grade 1, two had started out withonly 1 pronuclei and, while considered abnormal, were still cultivated and ended up being Grade 1s as well. Their chance of success was limited, but the embryologist suggested we use them anyway.

    We had the normal grade 1 and an abnormal grade 1 transferred, and I am now 11 weeks pg with a singleton.

    Do I think blasts are better? I cannot definitively say that that's what worked, or if our number just came up, but it seems like an awfully big coincidence to me.

    As for your P4 support, have you asked for the levels from your previous cycles? Armed with these numbers, you'll be able to get your doctor to explain how Pregnyl would be beneficial as compared to Crinone (yuk btw) or pessaries.

    Either way, good luck with your upcoming cycle! There are some girls in here who've run the gamut of medications, tests and protocols, and hopefully they'll have some answers for you too.

    Cass, I've got all blasts in the freezer, doing my second HRT FET now.
    I had Pregnyl support shots when doing my IUIs and IVF, I think it is my clinic standard, and also had to have pessaries as well for low progesterone in a couple of cycles.
    My clinic does a follow up BT to check hormone levels a few days after ET, & guess then adjust accordingly. Can't say anything specifically, because I just do what they say & hope for the best. But I do feel that they do cover all bases.
    It seems I have low hormone levels so they do keep a check on that & adjust whenever.

    I had my specialist appointment today and I discussed my concerns regarding my next course of treatment, he had decided to up my injections and I cant remember what they are to 200 a day plus he has agreed to give me an injection of HCG and crinone for support after the transfer. He has agreed to go to blastocyst but said that I will probably only get small numbers of eggs each time, I am 33 years old, it makes me quite anxious considering I was in the waiting room and this lady was talking about the 43 eggs she has on ultrasound, he told me I was on the verge on OHS and I only had 6 eggs go figure. Anyway thanks for all your replies I will keep you informed of my progress just waiting for AF to arrive now.

    Similar to you Cass I was on the verge of OHSS last cycle (& the cycle before actually was and had to be cancelled at OPU). I had 48+ follies and ended up with only 3 embryos. I had one transferred, which didnt stick and am now in the waiting stages of a HRT FET. I am on 4 progesterone pessaries a day as my levels are also very low. Previous cycle used the Crinone twice a day, but was just not enough.

    Hang in there and make sure the docs listen to your concerns. I really do think that a lot of the time we understand our bodies better than we realise.

    BTW, my clinic also only does blastocyst.

    Thanks Keen As I agree we do know our bodies well especially after what we all go through I think a lot of the problem is because I am a health professional they dont take me seriously they think that I am just being neurotic most the time but thats what they said everytime I told them I thought I had an ectopic, each time I go back to the clinic I throw something at them that I have researched or learnt of this site, I think it shocks them sometimes.

    lol Cass, I suspect my doctor. too , lamented me ever finding the internet!

    I find doctors tend to be much more leery of doing a blast culture if you don't start out with many eggs. I have never had more than 6 embryos fertilise, and my doctor kept discouraging me from blast transfer, saying there was a chance I wouldn't end up with anything to transfer.

    But hell, I'd done all the other rounds previously, and I needed to know if any were surviving to blasts. It came down to me being fed up enough to take the risk that we'd not end up with many, if any. Many clinics now do blast culture as standard procedure, anyway, so the success of them once you get to transfer must warrant the use.
