There are some great articles about birth at Birth Articles. Specifically, the article Birth Plans - Is It Worth Writing a Birth Plan? contains a link to a birth plan template.

Also, if you search the pregnancy/birth forums, I'm pretty sure there are threads where members have posted their own birth plans. If not, you could always start one asking people to share theirs . That's the beauty of BB, we all help and support one another.

A book I found really helpful was "Creating Your Birth Plan" by Dr Marsden Wagner.
