Whatever you do, make sure you mix the cocoa and sugar in a touch of boiling water before you add it to the milk. It needs to dissolve into a watery paste otherwise it wont properly mix into the milk. And dont let it boil!!!

Try adding a touch of chilli powder mmmm.
Cocoa, sugar, chilli, a tiny bit of water bring to the boil. Take off the boil and add milk. Heat until it is too hot to put your finger in.
Drink.
If you add about half a tsp corn flour with the water and then stir all the time it thickens. Then you get the Spanish style hot chocolate *drool*.
ooor....heat the milk before adding the cocoa and sugar- if it's just 'normal' cocoa, I use equal parts cocoa to sugar in full milk, lots of both... nice and sweet. Oh, and as N2L said, boiling is bad!!! Maybe add some cinnamon???
The best hot chocolate I have ever made was melting a block of dark chocolate in a pot, and stirring through a pot of cream, then as it warmed stirring through some sugar til it dissolved. Oh. My. God. Richest hot chocolate ever!!!!
