I have used this with grade 5/6 students and got some interesting results!
Format is as follows!

I am (two special characteristics)
I wonder (something you are actually curious about)
I hear (an imaginary sound)
I see (an imaginary sight)
I want (an actual desire)
I am (the first line of the poem restated)
I pretend (something you pretend to do)
I feel (a feeling about something imaginary)
I touch (an imaginary touch)
I worry (something that really bothers you)
I cry (something that makes you very sad)
I am (the first line of the poem repeated)

I understand (something you know is true)
I say (something you believe in)
I dream (something you actually dream about)
I try (something you make an effort to do)
I hope (something you actually hope for)
I am (the first line of the poem repeated)

I wonder if there is something else I should be doing
I hear the echoes of a fabulous future
I see delight in my baby's face
I want to be joyful in all things
I am loving and amazing

I pretend there is no housework
I feel there really are fairys
I touch peoples lives
I worry about the environment
I cry at the thought of losing my loved ones
I am loving and amazing

I understand life can tough in indescribable ways
I say do your best and have no regrets
I dream about my future
I try to be positive
I hope my daughter will be happy and carefree

I want a healthy and happy family to surround me

I feel the winds of time brush by me

I worry about the state of the world
I cry when I think I might lose what I have

I understand my husbands love for me
I say everyone's life is special
I dream about our lives in ten years
I try to be compassionate and giving
I hope my daughter does not struggle too much in life
