    On this form it gets lodged to Medicare.....so id assume it would end up in the right place....its on their government site!

    you dont need anything for chicken pox one even though its on scheduale it is still a optional one IYKWIM!!! we got Jack vacinated but didn't get the chicken pox one we took him for 18mth check etc, anyway about 2 months later I got a letter stating I was getting the immunisation payment etc, I never filled out any forms or sent in a letter etc

    yeah, for MIA it's fine - we're more looking at the details for CCB Rach.

    and in Kim's case, given that she's been toing and froing about whether to immunise, she may as well get the form filled out now...

    Another thing BG....i dont want the chicken pox one for Chelsea....but the later ones (like 4 yrs old ones) i will do....with sending in this form 'excuse' her for the chicken pox....but still allow me to do the ones down the track....

    DH said if it 'excuses' her for all of them thats ok...but cant stop me from still getting them even if the form said 'dont want immunisations'

    he's right hun - it will excuse you from all future immunisation - however, you can still make the choice to have them immunised if and when the time arrives
