Sunshine - you may have already done this but have you read the No Cry Sleep Solution?? I know you have already done the good start with the same night routine etc but there were a few other points that really made a difference for us (pre-April 2010 ) For us - we were missing the early tired cues and we have little beings that find an amazing second wind that just doesn't stop for hours!!!!! They run around like lunatics and that is the sign that I missed their early cues. Now, I just have to ride the waves until they settle (DD has just gone to sleep at 2230hrs) and DS1 is still up despite not having had a nap today. But they were / are overtired because I don't have the hands to settle them at the time they need it because DS2 is unsettled.

So I guess my point is, the book gave me the tools to recognise what their really early tired signs were and (for my children) I have a really small window of opportunity to settled them otherwise they reignite into the overtired, overstimulated state.

The Ashfield Infants Home has some services too if that helps but I am not sure about their techniques.
