Poor thing, head injuries are scary. Nurse on call have always told me to look for indentation and as much as it hurts them a bit to have a feel around and if it gives way to take them in to ED. I think the better out than in is true. I have seen a shocking egg on a kid at MG - it seriously looked horrifically scary and turned out to be nothing but just a bump.
