My MW asked me to push with a slight raised voice too. I could hear her trying to sound normal. I knew she's the last person to fret for no reason so I trusted her it had to be done. I didn't feel any anxiety though- just as you described. I could totally relate to that part.
I barely remeber why now. They didn't tell me until afterwards - his arm was coming out before his shoulder I think. When I look back at photos, I can see his hand poking out next to his head, before his shoulder is out. Do you know what it was with you?
