We (early intervention service) send all our school starters off with an 'about me' card.
It has a pic of them and just says something like: "hi, I'm XXX. I really enjoy XXX and love to talk about XXXX. Sometimes I get anxious when XXXX I show I'm getting anxious by XXX. You can help me by XXX. Other things that might help me when I'm stressed are XXXX."
You get the idea. We try to keep it as short and simple as possible so that even a really busy teacher will take the time to read it. But make sure we include the major important points to best understand and support the child so that when (s)he is not with the key teacher who knows him/her well, that relief teachers, the PE teacher, music teacher etc. can all get to know the child too.
Even if this doesn't end up being part of your communication book, it may be a helpful thing to do and distribute amongst the staff who will be in contact with your DS
