It sounds like a few issues TBH. If the care in the village has gone down, she may be scared to go back, and this is manifesting in physical pain. I definitely don't think she wants to go back, but I don't get the impression from what you've written that she's doing it deliberately. On that front, you can call the Health Ombudsman and look into a surprise 'inspection' by the authorities to check up on the facility which may give you a clearer idea on how she lives and why she may be reluctant. It also sounds like there's some dementia creeping in there, given her age. Some people regress as part of the dementia, which could also lead to her being afraid of being by herself.
