I just got my bill from MIVF for my current cycle and they have included a new schedule of treatment costs effective from 1st July - of course they have gone up! I have included the out of pocket costs after the safety net has been reached in brackets.

Patient admin fee $230 ($230)
Stim IVF cycle $2150 ($656.92)
Stim IVF with ICSI $2527 ($732.32)
Natural IVF cycle $1350 ($496.92)
Natural IVF with ICSI $1627 ($552.32)
Thaw cycle $1035 ($207)
AIH/IUI cycle $500 ($100)

Hospital bed fee RWH $308 Epworth Freemasons $740
plus anaesthetic fee
plus additional drugs

My PHI covered all costs for EPU and refunded $50 per bottle of synarel.
