    E2 Levels IVF Cycle

    Hi,

    I am doing my second IVF cycle after my first one got cancelled before EPU due to really high E2 levels (24 000) and too many follies (50 on a dose of 200 Puregon). My new dose of Puregon is 100 and I just had my CD4 level E2 come back at 202 which seems on the low side. I am hoping that I am not getting the opposite spectrum of response this time :-(

    Can anyone share their IVF E2 levels from previous cycle experiences and also does anyone know what the unit of measurment for E2 levels in Australia is? I was researching information but most of the info is American or Canadian and I notice America measures in picomoles and Canda measures in picograms and these different give very different target levels. I wonder whether Aus is picograms or picomoles so I can compare?

    Any help hugely appreciated.

    24000! That's enormous!

    I had 14480 before my EPU, and had OHSS pretty badly. All the embies went to the freezer and we didn't transfer. And I don't think it's something I ever want to go through again. I have never been so sick or in so much pain in all my life.

    I'm afraid I can't help with the units, I'm sorry.

    But with regards to starting out with lower levels now... They've already seen you hyperstimulate pretty badly and they would be doing everything they can to avoid that happening again. You may find that they will gradually increase your dose and keep watching you closely to make sure things don't get out of hand. It's easier to increase slowly than to decrease the dose when things are already heading out of control. I was started on 15IU of puregon, had very little response (I think my first level after 5 days of injections was 280), was then increased to 300IU, started hyperstimulating and was cut to 200IU, 100IU, 75IU, etc, and my final puregon injection was withheld. The night before my trigger I only injected lucrin to try to keep things low.

    Thanks for the reply and sharing your story. You are absolutely right because when I asked the clinic if it was too low and whether I should be worried they said exactly what you said - they prefer to start me slowly and up it if needed rather than risk the total bust that occurred last time. Apparently it is also better for egg quality to start slowly and increase the dose than to start too fast and back off.

    Last time they checked me on CD3 and then not again until CD8. At CD 3 I was 458 (which was in the normal range I am told) but then by CD8 I had rocketed to over 10 000. So, they reduced my Puregon to 150 but by CD10 I had managed to get to 24000 and was feeling pretty terrible and so was cancelled L At least cancelling avoided what would have inevitable landed me up in hospital with OHSS had they done an EPU though, so I guess I knew it was the right call.

    Anyway, so we're back to the drawing board at half dose (100) and am hoping I start to take off a little so that by the time I do the BT and U/S on Friday (CD7) things are looking good. If not hopefully it isn’t too late to up the dose and then monitor me closely to make sure I don’t start taking off exponentially again. It is such a guessing game hey!
