I haven't been able to cut back as we need the money to pay for IVF! However, I have been lucky in that my boss is flexible. She has just asked me to keep track of my hours and work it all out at the end, if I owe hours or want unpaid leave etc.. Thankfully I have been able to get early morning appts at the clinic (which is only 10 minutes from work) and am at work an hour early on those days so I am ahead in hours if anything. When I go in for pick up/transfer I will take sick leave for that. I just put my head down each day and concentrate on getting to 5pm!
