I didn't get mine until post-birth and then it was the size of at least a fifty cent piece... largely uncomfortable and made me not want to move for the first week or more after birth.

I don't *think* it would be anymore difficult to give birth with one, just a high likelihood that you will create more I would assume. Sorry I can't be of much help.
