The hygiene issue isn't an issue nowadays, but the fact remains, when hands are washed, disease doesn't spread. It took a while for them to associate hygiene with epidemics back then.
Being exceptionally clean is no good either. You body does not get the chance to build up immunity to everyday viruses and pathogens.
