Cat - yup love my little squirt. Sometimes you can get them cheap off ebay. I got mine through Nurture nappies - they have them onsale.
You can use bum cream if you use a liner. Cheapest is buy some microfleece - cut it up yourself and use them. There are products out there too that are safe to use with cloth - most nappy sites sell them too
I've only just got one and am still getting used to it. I find it better than scraping off with an old spoon which I did before!

When C gets rash I use flushable liners and don't get the bunged nappy problem.
Little squirts are awesome. Especially for newborn poo which is like glue. I know plenty of people who have survived perfectly well without a LS, but I don't know anyone who has had one and not thought it was well worth the money.
For rash, like Mel said - liners. Only problem is (and my poor BumGs can testify to this), is that they can slip and slide around, so they are good for light smearings of cream, but if it's a serious bout of teething rash and you seriously have to slather it on, then that's the only time I ever use sposies.

Having had a bub who had runny poo for a VERY long time, i am sooooo grateful for our Little Squirt. DH and i both agree it's the best baby product we bought - and without it, doing MCN would be much less do-able for us personally. Some people are fine with dealing with the poo other ways, good luck to them. But the Little Squirt is the answer for us.
Do take the time to read the instructions, it's not the kind of gadget to use without learning HOW TO first.
regarding the nappy rash creams.
there seems to be two types.
the ones that are petrochemically based, the barrier creams. Yes, these interfere with MCN, and if you use them, place a microfleece liner in the nappy first, so the liner catches the cream, NOT the nappy.
the ones that are NOT petrochemically based. Simmons Paw Paw Salve is one i can think of that fits this catergory. (But Lucas Paw Paw Salve IS petrochemically based).
The Little Squirt is still a godsend to us 2.5 years down the track and with #2 on the way I think it was definitely worth the money. It's also great for cleaning out the potty into the loo so it's multi-functional
Whenever we needed to use rash cream I got the biodegradable flushable liners although this time I will look into the reusable ones.
