The internal pressure is probably from further hemorrhoids inside your rectum. It sounds like you have a pretty nasty case! I did too, you are not alone. My OB prescribed me with an ointment called Sheriproct. It is heaps stronger than Rectinol or any of the other 'over the counter' ointments. It sorted out my 'roids within a week or two. Only problem is that you need a prescription for Sheriproct and it is not on the PBS (it wasn't a few years ago, maybe it is now???) so it's expensive, about $65 for a smallish tube. Well worth every cent I say, and a lot cheaper than surgical removal!!
Sheriproct ointment was fantastic! I've used other types before for tiny roids but the Sheriproct was the best!! They are horrible I had big bad ones and all the symptoms you're describing. The other thing I did was ice, if you get a little flat ice pack you can put it inside a pad I found that helped.
I feel for you the were horrible I couldn't drive until mine went as I couldn't sit properly...but they do go!
