    Just wanted to ask... do you still receive the Mat Immunisation Allowance if you sign this form instead of getting the vax's???

    Yep, that's all you have to fill out. We just did it a few weeks ago. Our GP was fine with signing it and didn't say too much coz we've already talked to her about vaccination before.

    I think you have to get it in before they turn 2 to get the payment though... not sure, I just wanted to stop the stupid letters.

    I got my form yesterday. If I turn it in to Medicare, will that keep Centrelink happy? Or will I have to provide them with their own copy? (Although, they're obviously sharing info since I'm getting letters).

    Also, am I required to provide a copy to DD's daycare provider? I probably will anyway, just out of courtesy.

    I have to laugh at the fact that I'm getting letters warning me that I might have to pay full child care fees. I do anyway because I'm not claiming the CCB as reduced fees. Ooh, I'm so scared

    (Sorry to hijack Shanti, just thought these questions may as well go here rather than starting a new thread).

    Yeah, just one to medicare is fine. That's all we did and then we got a letter from centrelink.
