Can you combine nurofen plus with panadol osteo (which is slow release)? Or is there a slow release codeine out there?
It seems as though your biggest issue is that it doesn't last long enough, rather than that it's not strong enough.

Also, consider topical anaesthetics - even teething gel might help a bit, but I think that there are stronger ones available too.
