I don't think it's a matter of having to hitting the pavement and doing a 10km run straight up.
When I first started training, I couldn't run around an oval without stopping 5 times. I couldn't believe it. But you have to start somewhere. My fitness started to increase but I didnt start losing real weight/size until I altered my eating habits and stopped eating so much junk. Remember, if nothing changes, nothing changes.

I once read about a woman who was morbidly obese who started just walking around the Hills hoist in her backyard. For someone carrying her weight, that was enough to start without risking her life. Combined with healthy eating (both in quality and quantity), she lost heaps of weight. I think this way, you put so much work into losing the inches, you're less likely to indulge in things that will compromise all that work. Also, the healthier you become, the better you feel and the less you want to stuff the emotions with food, IYKWIM.

I think you need to talk to a professional - a qualified dietician. Not just someone with Cert 3 in personal fitness. Someone who really knows their stuff and can work with you and your doctor on a realistic program.
