I was careful but not overly careful in my first pregnancy. I ate sushi but stayed away from the cooked chicken sushi and raw fish varieties. I usually ate the tuna salad variety which has tinned tuna in it. I also made sure I bought it from a reputable shop that was busy. In saying this, a friend of mine got sick from eating vegetable sushi (she was not pregnant) so vegetable sushi isn't exactly guaranteed to be safe.
It is very easy to go overboard. At one point I found myself thinking that I could not even eat a salad sandwich from a sandwich shop because they use the same tongs to put ham etc on sandwiches and then use them for the salad.
I think it's a personal choice. It is a balancing act.
