Your middie will see them Otherwise just let them know. To my knowledge there's not a whole lot you can do. I got haemorrhoids during DD1's pregnancy, they blew out to two golf ball sized ones during the birth (but admittedly I was pushing for 1 hour and 40 minutes), then eventually went down and inside again, then came back during Dd2's pregnancy. Didn't blow out as bad with her birth at all (6 mins pushing in water) but still uncomfy for a while. TMI...now every time I have a movement they come back out -_- I guess eating lots of fibre, drinking lots of water, using gravity for pushing and staying active during labour, will all help to some degree.
