I suppose it works differently everywhere. For me, I went and saw a doctor when I first thought I was pregnant, and she referred me to the antenatal clinic at the local hospital. I received a letter in the mail telling me of my first "booking in" appointment, which was at about 12 weeks. Because I have all my check-ups, etc at the hospital, the booking in is kind of automatic. I think it probably works differently at different places, and probably depends on what kind of care you have - GP, midwives, etc. If you're worried, ring up your hospital and just ask them what the procedure for it all is.
